The Graham Wellness Center is located at 235 West
Walnut in Canton, across from the Graham Hospital.
Currently housing the Graham Hospital
Cardiopulmonary Rehabilitation
program, participants enjoy a spacious, 5000 square
feet facility equipped with lockers and individual
showers. A wide variety of exercise equipment, free
weights, and warm up/cool-down areas are also
available, as well as a
state-of-the-art kitchen/classroom used for education
and food preparation demonstrations.
Focusing on both prevention and rehabilitation, the
Graham Wellness Center offers several cardiopulmonary
rehabilitation options.
Regardless of the program chosen, participants receive
personalized instruction on equipment usage from
professional nurses and exercise specialists. Participants
give feedback to our staff on their response to the
exercise program. Exercising in our clean, safe,
state-of-the-art facility is uplifting and enjoyable.

Phase I Programs: Inpatient Cardiopulmonary Rehabilitation
The Wellness team will consult on inpatient cardiopulmonary patients for
assessment, education, and low level exercise.

Phase II Cardiac Rehabilitation
This 8-12 week program meets three times a week at the Graham Wellness Center. Heart
rhythm and blood pressure are monitored each session, and exercise progress is tracked by
trained staff members. Nutritional assessments and education about coronary risk factor
reduction and stress management is included.

Requirements
- Heart attack, bypass surgery, chest pain with exertion, newly covered diagnosis from Medicare,angioplasty with or without a stent
- Valve repair or replacement

Phase II Pulmonary Rehabilitation
Persons suffering from asthma, emphysema, chronic bronchitis and restrictive lung disorder are
candidates for this program. Lasting up to 10 weeks, participants receive heart, blood pressure,
and oxygen level monitoring. Education for optimal self-care is emphasized.

- Pulmonary Function Test within 3 months of starting
with levels that meet Medicare requirements

- If smoking and on Medicare, you must take a smoking
cessation class or quit before starting the program.

Freedom From Smoking
Freedom From Smoking is a six-week smoking cessation program
available to the public as well as our participants. An American
Lung Association Program designed to help you quit with support.
Freedom From Smoking is $124.
Almost everyone who is asked expresses regret about starting
to smoke, and everyone who quits is glad that they did. Why then,
do we continue to smoke even though we know it is bad for us?
Addiction to nicotine seems the obvious answer and it is a formidable
foe. But smokers also express fear of failing as the factor that
keeps them smoking. Many have attempted to quit unsuccessfully
in
the past, and believe that resuming smoking may be more painful
than facing the health toll of continued smoking. Smoking often
fills
emotional needs and is used as a stress tonic, which complicates
the habituation.
Smoking is something that we practice over and over again, every
day. Quitting is more than just a smoking cessation program.
Your smoking motivations and patterns are identified. Concrete
tools for quitting are incorporated into your plan for stopping
and how
to stay free is reinforced. You are not alone in your effort, and
you have support after you quit.
The Freedom From Smoking Program is offered periodically throughout
the year by smoking cessation leaders trained by the American Lung
Association. You can participate in this program as a component
of the Prevention Program, or just take the smoking cessation

The Prevention Program

Cardiopulmonary Rehabilitation
Persons wanting exercise supervision or those just completing Phase II cardiac or pulmonary rehab can maintain fitness in a
secure environment indefinitely. Exercise classes are held two to three times a week. Weekly blood pressures are taken and
either monthly or bi-annual telemetry monitoring is provided. This affordable self-pay program encourages long range adherence
to exercise.
The Prevention Program
An eight week self-pay program will be offered for patients who have been identified by their
physician as being at high risk for developing coronary disease, but who have not suffered a recent
coronary event. This program offers individually designed exercise programs, nutritional and lifestyle
education, and weekly cardiac monitoring.
PAD (Peripheral Artery Disease) Program
If your legs ache when you walk very far or if you have been told that you have poor circulation or a blockage in an artery in your leg, the PAD program can help you.
This six month program is specifically designed for persons who have poor bloodflow to the legs.
Patients recieve intensive risk factor and exercise trainging for the first two months and then continue
training for an additional four months for maximum benefit.
The training program differs from our other programs. Education centers on reducing risk for artery disease. Doctors order required. Some are covered under insurance. Self pay is very reasonable.
