Patient Registration

Patient Registration is located on the ground floor near the Lobby. They are
open from 5:30am - 8:00pm Monday through Friday, 7:00am - 2:00pm on Saturday,
and 7:00am - 1:00pm on Sunday. All patients who receive hospital services must
go through the Patient Registration area.
To register for hospital services at times other than those listed above,
please go to the hospital Information Desk located in the main lobby.
Business Services

Business Services is located on the ground floor near the Lobby. They are
open from 8:00am - 4:30pm, Monday through Thursday and Friday from 7:00am -
3:30pm. The staff is available to answer questions concerning your hospital
account.
  
Need Financial Help?
Graham Hospital is proud to serve the residents of Fulton County with high quality, accessible, cost effective care. Graham Hospital accepts all patients without regard to race, color, creed, or ability to pay.
Financial assistance is available for Fulton County residents for medically necessary services to patients meeting financial criteria. If you do not have health insurance or are concerned you may not have enough insurance to cover the costs of your services, and you are a Fulton County resident, financial assistance may be available to you. We may be able to assist you by:
- Helping you apply for Medicaid
- Determining eligibility for free or discounted health services
- Establishing a payment plan
Through an interview process and with financial documentation we will ask from you, we will be able to determine eligibility for appropriate programs where you may qualify for assistance. We will need information from you about your job, income, resources, insurance coverage, family size and other information to help determine appropriate programs for you. This will include your most recent IRS 1040, most recent W-2's, recent paycheck or unemployment stubs, Illinois Department of Public Aid Approval/Denial, Social Security or Retirement Benefits statements, most recent bank statements for checking and savings accounts, most recent property tax bill for Home Rental Property and/or farm or other supporting documents. A financial assistance application must be completed to determine your eligibility. You may request a financial assistant application by calling our financial counselor at 309-647-5240, x2213, or you may print the financial assistance form, fill it out completely and mail, along with the requested financial documents to:

Disclaimer
Graham Hospital reserves the right to reverse any charity assistance in the event that you falsified data or failed to disclose financial information on your application for charity assistance. If there is a pending liability claim, workman's compensation claim, or insurance claim, charity assistance cannot be applied, or may be reversed at anytime. You must inform our Business Office, in writing, of any changes in your financial circumstances affecting your ability to pay any balance due from you.
Health Information Management

The Health Information Management department is open from 8:00am - 4:30pm,
Monday through Friday. The staff is available to help with requests regarding
your medical record. They are located on the lower level.
