Graham Hospital’s Extended Care Facility is a 38 bed nursing care facility located on the second floor of Graham Hospital. Medicare certifies 20 of the beds for skilled care. All beds are accredited by the Joint Commission on Accreditation for Health Care.

The facility offers private, semi-private, and three/four-bed rooms. All rooms have a half or full bath, color-cable television, personal storage area, bedside nurse call system within the TV remote, and telephone.

It is our belief that each patient/resident should be assisted in achieving his/her optimal level of wellness. Our interdisciplinary team works together to develop and follow an individualized plan of care for each patient/resident.

The words “hospital and “hospitality” are both derived from the Latin work “hospitalitas.”

The employees of Graham Extended Care welcome our guests with warmth and respect, the true meaning of hospitality.
